At REMSA Health, Mobile Integrated Health (MIH) is built on a simple idea: some community members need more than a response to an emergency. They need ongoing support, care coordination, and connections to the resources that can improve their health and quality of life. MIH helps bridge those gaps by meeting patients where they are and working alongside healthcare providers, social service agencies, and community partners to address the factors affecting their well-being.

Recently, the MIH team celebrated the graduation of a patient we’ll call Luis.

Luis was a charismatic entertainer who spent much of his life bringing joy to others. As he grew older and lived alone, cognitive decline began to affect his memory. Over time, inconsistencies in important documents, including his Medicare information and Green Card, disrupted his access to healthcare benefits and social security income. Eventually, those challenges led to eviction, leaving Luis without stable housing, income, or reliable access to care.

Luis was referred to REMSA Health’s MIH team by REMSA Health AEMT Tatum M. at a time when he had few places to turn. For some time, 9-1-2 was his only source of support and human connection. REMSA Health Community EMTs Caroline H. and Grace K., working alongside partners from Washoe County Case Management, Washoe County Senior Services, and Access to Healthcare, helped coordinate the services Luis needed to rebuild stability in his life.

The team connected Luis with a mobile dementia support specialist, assisted with gathering critical medical documentation and helped coordinate and attend primary care appointments where they served as advocates for his healthcare needs. When it was time to transition into an assisted living facility, MIH was there to ensure a smooth move into what will hopefully be Luis’ forever home.

Stories like Luis’ demonstrate why REMSA Health values Mobile Integrated Health as an essential community program. By addressing healthcare needs before they become emergencies, MIH helps patients achieve better outcomes while reducing strain on emergency services.

The impact in Luis’ case was significant. In the six months before enrolling in MIH, he activated 9-1-1 services 20 times. In the six months after enrollment, that number dropped to just two calls, representing a 90% reduction in 9-1-1 utilization. More importantly, Luis gained stability, support, and a pathway to ongoing care.

For the MIH team, Luis’ graduation is more than a milestone. It is a reminder that when healthcare, social services, and community partners work together, lives can be changed one patient at a time.
